How to Fill It Out

  1. Enter employer EIN, name, address, and employee SSN and name.
  2. Report wages, tips, and allocated tips in the appropriate numbered boxes.
  3. Fill in federal, Social Security, and Medicare tax withheld amounts.
  4. Complete state and local boxes if applicable, then review all totals before printing.
